How to send the Dom Care SitRep survey

Process - Send Dom Care Sit Rep report

1. In the Care Bridge menu choose Send forms

2. In Send forms you set up all of the information required and choose Domiciliary Care Provider Survey as the assessment tool. Add the relevant recipients

3. Once you have completed all of the information click "send".

Process - Review Dom Care Sit Rep Form status

In the Care Bridge menu choose Review forms

2. In Review forms you can see a list of all the requests you have sent and their status:
   • The form sent
   • The service user it related to
   • The recipient of the form
   • Who sent the form
   • When it was sent
   • last updated date
   • Status
   • Who contributed to the from completion
